Patients can now request appointments online! Why wait on the phone when you can message us online instead?
Click on the “Patient Login” button below to get started. Once you arrive at the login screen, simply click on “create an account”. If you have registered with our practice, or any practice that uses Healthkey, you can just enter your user ID and password. Once you are registered, you can request an appointment with your doctor, at the times and days of the week that work best for you. |
 Patient Login offers patient services online through an interactive secure web portal. The secure web portal is designed to provide you with easy access and a simple format to communicate with your doctor. You can now pre-register and complete your health history forms by submitting them online once you have logged into the portal and created an account. Other online services coming in the near future are requesting appointments, prescription renewals, and paying bills. Registration is FREE, it's easy to use, secure, confidential, and can be accessed from anywhere 24/7. To start, simply click on the patient login button above.

Announcement: Dr. Shore is currently accepting new patients. Dr. Shore now has appointments until 7:00pm on Monday nights as well.
Note: A physician is on call 24 hours a day, 7 days a week for urgent medical needs for our patients. When the office is closed, you can speak to the on call physician by calling (734) 973-4800 and following the instructions. |
